For those of you entering our Futurities this year you will have to have an OTR futurity Form with your show entry form as well.  Below is the additional form that you will need WITH a COPY of you papers!

PLEASE go to 7-11, BP, Kwik Stop, The UPS Store wherever you need go to get copies.   DO NOT email it at the show or Text it BRING A COPY OF THE PAPERS!  THANK YOU and that goes for the ECSIF horses as well!
